Botox and Fillers Are Very Safe, Brookline, MA

Joseph Coupal - Tuesday, November 11, 2014

Cosmetic procedures such as Botox, facial fillers and laser skin treatments are very safe, and have a very low rate of minor side effects, a new study suggests.

These cosmetic procedures can cause minor side effects such as bruising, lumpiness or skin discoloration, but these occur in less than 1 percent of cases, according to the study published today (Nov. 5) in the journal JAMA Dermatology.

"We strongly suspected the rates were going to be low, but we were pleasantly surprised that they were as low as they were," said study lead author Dr. Murad Alam, a dermatologist at Northwestern University in Illinois. (In past work, Alam has conducted studies that were funded in part by Allergan, Medicis, Ulthera and Bioform, which are companies that sell products for cosmetic procedures.)

However, the study included only dermatologists who routinely perform these procedures, so it's possible that negative outcomes could be more frequent in the hands of less-experienced providers, said Dr. Michael C. Edwards, the president of the American Society for Aesthetic Plastic Surgery. [7 Plastic Surgery Myths Debunked]

Also, the researchers did not collect data on whether people were satisfied with their appearance after the procedures. Even if they don't experience bruising, swelling or lumpiness, it's still possible that people simply don't like the way they look after cosmetic procedures, Edwards noted.

Low risk

In the past several years, minor cosmetic procedures such as laser skin treatments, Botox and facial fillers have surged in popularity. Botox is aimed at smoothing out wrinkles, whereas facial fillers are used to plump up features that have been deflated by age or sun exposure. Lasers are used for removing hair, tattoos and spots on the skin, as well as tightening the skin. Unlike traditional facelifts, these procedures can be done without general anesthesia, and patients often need only a day to recover, Alam said.

Doctors suspected that the risks of these procedures were fairly low, but previous studies had looked at only a small number of cases, from individual doctors' recall of patients' side effects, after the fact . To more rigorously examine the rate of side effects, Alam and his colleagues asked 23 dermatologists throughout the U.S. to enter data on each cosmetic procedure they performed for three months, along with any adverse events the patients reported over the next year. All told, the team looked at 20,399 procedures.

About 1 in every 416 procedures resulted in a negative outcome such as bruising, swelling or lumpiness in the skin. But some procedures were riskier than others. Procedures involving neurotoxins, such as Botox, caused side effects in just 1 in 3,333 cases, whereas fillers such as Juvederm caused lumpiness or beading in about 1 in 135 cases.

The low rate of side effects may make these treatments an attractive alternative to a facelift, which requires cutting the skin and going under anesthesia, Alam said.

"By doing lots of small procedures we can avoid the risk of a larger procedure," Alam told Live Science.

Safety varies

Although the findings show these procedures are generally very safe, the researchers looked at board-certified dermatologists, Edwards said. But the safety of these procedures hasn't been studied in less- experienced hands, and the procedures can still cause real harm, he said.

For instance, using a high-energy laser at the wrong setting can block blood vessels and cause tissue death, he said.

"The strip malls and the storefront mini-spas and places where there's no true oversight — that's where patients should be more cautious," Edwards told Live Science.

I Got Better Skin During My Lunch Break - Brookline, MA

Joseph Coupal - Tuesday, November 04, 2014

What really happened when one editor tried out the noninvasive resurfacing laser treatment called Clear + Brilliant. The cost, the procedure, and the results she experienced, from taut hotness to "Oh, how I glow."

Procedure: Clear + Brilliant

What it is: A noninvasive resurfacing laser treatment that promises to even skin tone, boost radiance, reduce pore size, and enhance elasticity by drilling microscopic holes in the surface of the skin and kicking collagen into healing overdrive. Clear + Brilliant is billed as being preventative—according to proponents, a series of once-a-month sessions can keep early signs of aging at bay—as well as restorative. Derms love it because it can deliver Fraxel-like results after only a single treatment, patients love it because it doesn’t render them social pariahs for a week afterward.

The cost: $300 and up.

Editor’s take: Laserwise, I don’t think I’m ready for the big guns, but my skin has been looking decidedly blah lately, a result of frequent air travel and a noncommittal relationship with daily sunscreen. I didn’t enjoy much outdoor fun this summer, but a whole coterie of little brown spots have popped up on my cheeks, eager to join the party nonetheless. It’s time for some zapping, so I make haste to Dr. Brandt Dermatology Associates in New York City.

First, a nurse applies a topical numbing cream that looks like cake frosting (but tastes, I accidentally discover, like bitter, mouth-deadening sadness); once it’s kicked in, dermatologist Robert Anolik, MD, begins the treatment, gliding the small Clear + Brilliant handpiece across my cheeks, forehead, nose, and chin. The sensation is weird but not unpleasant—it feels sort of like there’s an electric cat tongue licking my face—and it’s over in about 10 minutes.

The results: Immediately after the session, I look and feel like someone who has been way too enthusiastic about roasting s’mores: My face has the same taut hotness that you get from sitting too long in front of an open flame. After a few hours, the heat dissipates, but the redness, and a very mild swelling, remains overnight. For the next three days, my skin feels sandpapery (although strangely not dry) but looks amazing: The discolorations are already gone, and oh, how I glow. There’s no peeling, no blotchiness. By day five my face is noticeably springier and plumper, clearer and more radiant than it has been in months, if not years. Sold!

Dream Spa Medical’s Clay Mask Facial: Using nature to achieve soft smooth skin - Brookline, MA

Joseph Coupal - Thursday, October 30, 2014

By: Mary Kelley

One of my favorite masques from SkinCeuticals is the Clarifying Clay Masque. The namesake ingredient is a one-two punch of kaolin clay and bentonite clay. These two natural clays absorb excess oil and help remove impurities from clogged pores.

The clays work synergistically with an amazing blend of hydroxy acids designed to exfoliate the skin, leaving your skin soft and clear. The addition of aloe and chamomile soothes the skin. Suitable for all skin types, this masque really benefits normal, oily, combination, and acne/congestion prone skin. I love this masque even for my dehydrated skin because my nose and forehead is so clean and clear after I rinse the masque off; and, as a bonus, my skin isn’t dried out or flaky afterwards!

This light masque is a great treatment for use at home either used as a weekly masque or as a spot treatment for stubborn breakouts. However, the real magic happens when we use the Clarifying Clay Masque in the treatment room with our Clay Masque Facial. Clients who are concerned with clogged pores, oily skin, or acne love this treatment because the combination of clay and hydroxy acids leaves skin soft and pliable--perfectly prepping the skin for extractions.

The Clay Masque Facial is perfect for most skin types and conditions but is ideally suited for clients with acne, oily skin (visibly large pores), or pores that tend to become clogged. We use steam for clients who need it but the treatment is just as effective without steam, making it suitable for those who have broken capillaries on their cheeks or more sensitive skin.

Afterwards, your skin is clear, clean, and glowing. This is the perfect facial to kick off the cooler months with. You’ll be free of the oil and congestion that makes pores more apparent and you’ll have softer, smoother skin just in time for late fall/early winter events. Book your Clay Masque Facial with Dream Spa Medical today! Our bookings fill up quickly this time of year and we’d hate for you to miss out on gorgeous skin.

How to Wash Your Face - Brookline

Joseph Coupal - Thursday, October 16, 2014

The cold weather has set in, and cold weather means we have to treat our skin better. There are spa treatments that can help your skin stay healthy and younger looking and there are some at-home basics that can help too.

Spa Treatments for Winter Skin

HydraFacials are completely non-invasive and are a great treatment that removes dead skin cells and impurities while bathing the new skin with cleansing, hydrating and moisturizing serums. The treatment is soothing, refreshing, non-irritating and immediately effective. Microdermabrasion is a very popular skin resurfacing treatment. Most of our patients see amazing results after one treatment of medical facials or chemical peels. These treatments leave your skin looking smoother, younger, and more consistent.

However, you also need to know how to take care of your skin at home. Start with the basics, and make sure you are washing your face correctly every day.

Wash your hands first.

Here’s a rule that’s very easy to ignore: Wash your hands before you wash your face. If you don’t, you’re essentially just rubbing dirt into your pores.

Use the right water

Not all water is created equal: Hot water can cause dry skin, and really cold water can be just as bad. Choose lukewarm water always and you’ll start seeing results. If you wouldn’t drink the tap water in your house don’t use it to wash your face either. If it’s not good enough for inside, it isn’t good enough for your exterior either. You don’t have to cleanse your skin with bottled water, but it’s smart to boil tap water and let it cool before you wash.

Take off Your Makeup First

Many people wash their face and “take off their makeup” at the same time. Take off your makeup first. That way, your cleanser can work deep into your pores without mineral powder getting in the way.

Don’t rush

Most are guilty of rushing through routines, including skincare. Take your time massaging your cleanser into your face without scrubbing. When you’re done, rinse the cleanser off completely, making sure nothing is left behind. It’s similar to hand washing clothes: If you leave soap behind, it will surely get dirty faster.

Over-medicating your face with topical ointments and chemicals is a sure way to irritate your skin and enter a never-ending cycle of problems. (An acne treatment that leads to dryness which leads to an oily moisturizer that leads to acne which leads to…) One thing you can do to limit the amount of irritation on your face is to control the different products you put on your face. The ingredients in these products are totally safe to use… until you start to pile up several different ones, because chances are they haven’t been tested together. Buying all your products from the same line can reduce the amount of skin woes. you can always consult with a Dream Spa Medical professional about which products are best for your skin in particular.

Most people use their hands to wash their face. Fingers are gentler than a washcloth. While that may be true, you can’t deep clean like a washcloth could. You can also brush. After using one, you may notice a decrease in clogged pores. If you aren’t a fan of the time it takes to efficiently scrub your face with your hands then getting a brush is definitely the way to go — just be sure to pick one with soft bristles if you have sensitive skin. If you want a thorough clean, we recommend the Clarisonic facial cleansing brush.

Get Your Skin Ready for Fall and Winter - Brookline, MA

Joseph Coupal - Monday, October 06, 2014

It’s that time of year again! The kids are back at school, the leaves are changing, the harvest is in, and wherever you turn, there is someone counting down the days until the holidays. My neighbor knows exactly how many days it is until each of the upcoming holidays and tries to tell me every morning.

Her enthusiasm and proactivity has made me think about how we should be caring for our skin leading up to the cooler months.

Whether you need advice on changing your routine to accommodate the drier and colder air or if you want new treatments to look your best for the upcoming parties, reunions, and other events that mark the winter, Dream Spa is your one-stop shop for looking and feeling your best.

Let’s start with a timeline to make things less overwhelming and easy to follow:

Good to do: Switch your lighter moisturizers for something a little heavier but with an added boost of exfoliating glycolic acid. Your skin needs the extra moisture but after a long summer of humidity, you will also want to encourage cell turnover for brighter looking skin.

Better to do: Schedule a consultation and facial with one of Dream Spa Medical’s excelled aestheticians. Now is the time to go over your skin concerns that developed over the summer (forgot sunscreen a few too many times?) and start prepping your skin for fall and winter treatments.

Best: Do all of the above but begin working on pigmentation caused by sun damage or other factors with a series of laser peels or chemical peels. Both types of treatments are very effective and require little downtime post procedure. Ultimate: Really wow your friends and family by having an Ultherapy treatment. This non-surgical alternative to a facelift is a revolutionary way to turn back the clock. Using ultrasound, Ultherapy lifts and tightens your facial tissue to create a younger you. In fact, Ultherapy is the only FDA-approved ultrasound treatment to non-invasively lift the eyebrow, neck and under-chin. Now, FDA-cleared to improve décolletage lines and wrinkles as well. There is absolutely no downtime to this anti-aging procedure. Your facial and neck tissue will gradually tighten and lift over 3 months, with continued improvement over another 3 months, but you won’t have any of the downtime associated with surgery.

Next Month or two:

Good to do: Continue using the home care your Dream Spa Medical aesthetician has customized for you.

Better to do: Schedule a free consultation with your aesthetician for a progress check on how your treatments and homecare are working for your skin. This allows you and your aesthetician to make adjustments as needed.

Best to do: If you haven’t considered addressing any concerns regarding pigment, unwanted hair, or unwanted veins, now is the time to do so. The sun is not as strong and it is the perfect time of year to remove unwanted pigment, hair, or veins.

Ultimate: Make an appointment with one of our nurses for Botox or fillers. Being able to smooth out the frown lines on your forehead and replace volume loss around your cheeks, mouth, and eyes will make you look instantly younger. Imagine how nice it will be to look serene and relaxed during the stressful time after planning or traveling for the holidays!

End of the year:

Good: Schedule a consultation to assess your progress with your aesthetician. Discuss any upcoming travel plans to warmer climates and stock up on sunscreen.

Better: If you are a winter sports athlete, schedule a facial designed to soothe and hydrate the skin—such as the HydraFacial. The weather is changing again. The fall was dry and cold but the darker winter months can wreak havoc on your skin. Check with your aesthetician about any new skin concerns and ask what products can best resolve those issues.

Best: Check in with the nurse who did your Botox. Is it time for a touch-up?

Ultimate: Schedule a Botox Party or Spray Tan party with your friends. It’s been a long winter and you all deserve to relax and have some me time!

Call Dream Spa Medical today to start getting ready for the coming Fall and Winter.

Fall Skin and Beauty Tips for Fall – Brookline, MA

Joseph Coupal - Thursday, September 18, 2014

Fall is slowly but surely creeping up on us and with the new crisp air, here are five tips to keep in mind as you get your skin and yourselves ready for the new season:

Don't Hold Onto Summer. It is always sad to see summer end. With the end of summer it's time to retire certain beauty items. Bright, bold pinks, teals, neons, shimmery bronzers, and summer fragrances all need their annual time off. Continuing to wear these seasonal shades will have you standing out for all the wrong reasons.

Don't Deep Dive Into Winter. Just because summer is over, you can't go from wearing a coral lip color one day to a deep plum or oxblood the next. Choose transitional neutrals and classic reds until the cold really hits.

Don't Neglect Your Skin. Don't put that sunscreen away! This is not a seasonal beauty product! This is a skin care essential! Just because the air is cooler, doesn't mean the sun lets up. Don't Become A Sasquatch. This is tempting to do after shaving all summer long, but do your best not to let yourself become a hairy little monster this fall.

This is especially true if you wax. Waiting too long in between waxes means more pain when you go to wax later. The longer the hair, means a more painful pull. Besides, feeling silky smooth under all your fall and winter layers will make you feel super sexy!

Don't Buy All New Makeup. Tempting as all the new fall makeup collection are, don't overdo it by splurging on everything and anything.

Ready for fall? If not, contact Dream Spa Medical in Brookline.

Tattoo Removal Takes Time, But it Works – Brookline, MA

Joseph Coupal - Monday, August 25, 2014

That tattoo you wanted forever in your 20s might not seem like the best decision by your 30s. Many things in life change, but, ink doesn’t, unless they start to stretch or droop.

Tattoos are still popular. But, Americans will spend more than $75 million on tattoo removal this year. It used to be that tattoo removal also meant removing part of your skin. But, that’s not the case anymore.

Tattoo removal can help erase many bad decisions or lost bets. It is a process and it takes time. But tattoos can be removed. No blood is shed. The secret is a special laser that senses tattoo ink in the skin.

The molecules of tattoo ink are so big that the cells that are in charge of removing our skin can’t physically move them around. This is why tattoos are permanent. The laser doesn’t actually remove the tattoo, it simply breaks down the ink molecules inside.

The ink is broken down into small enough pieces that the cells in the skin can remove them. The ink doesn’t go away on the spot, rather it fades away over weeks, even months.

In most cases, it does fade to the point where you can’t even see it anymore. The idea is that we get as much clearance as we possibly can in as few sessions as possible.

The hardest part is predicting how long it will take for a tattoo to completely go away, because everybody’s body works differently.

For more information on tattoo removal, contact Dream Spa Medical in Brookline.
